Home Health
Home health visits are made by a Registered Nurse under the orders of a physician to homebound persons with a qualifying diagnosis. This care is covered by either Medicare, Medicaid, or private insurance. We work with DHSS in assisting with referrals to provide care. Services we offer include:
- Home health nursing
- Home health aides
- Physical therapy
- Occupational therapy
- Speech therapy
- Medication set-up
We also make prenatal and newborn visits with a prenatal case-manager to assist you through your pregnancy.
We are staffed with five full-time RNs to provide skilled nursing care. The Sullivan County Health Department is proud of our continuity of care, because when your eligibility for Medicare or Medicaid has ended, these same Registered Nurses will continue to visit you under Sullivan County's Public Health Program, paid for by your tax dollars.
